If you have a request for your records, please give adequate time and a written consent, especially if reports are being released to a third party. It is office policy that results be given only to the patient, with an exception being made for minors or those unable to comprehend (patients who may be mentally challenged). Results will be relayed to a third party only with the patient’s consent, in respect of patient’s privacy rights. Please inform office personnel if you wish results discussed with anyone other than yourself. Please note that there may be an administrative charge to copy records.
